The goal of this activity is to increase the capacity of diabetes educators, health educators, community health workers and others who work with people with and at risk for diabetes in community and clinical settings to implement evidence based education and communication strategies to improve diabetes prevention and self-management.
Diabetes Self-Management Education and Support programs enhance a variety of outcomes for people with diabetes—but too few patients are receiving DSMES services. You have the power to change that. Presenters will discuss the four critical time points to assess, provide, and adjust DSMES. These referral opportunities are described in "Diabetes Self-Management Education and Support in Type 2 Diabetes", a joint position statement from the American Association of Diabetes Educators (AADE), the American Diabetes Association (ADA), and the Academy of Nutrition and Dietetics (AND), created in 2015 with assistance from the National Diabetes Education Program (NDEP). The webinar will also highlight a toolkit created by these national organizations to assist diabetes educators and others with sharing the information and recommendations included in the joint position statement. Webinar speakers will unravel the mystery of DSMES though a role-play showcasing tips to use this position statement to increase referrals to DSMES services.
